Job Description

Insight Global is looking for qualified Case Managers to join the team of a client who is dedicated to providing comprehensive support and services to veterans in need. Their mission is to empower veterans to lead fulfilling lives by offering assistance with housing, healthcare, employment, and other essential services. The Case Manager will provide personalized support to veterans, helping them navigate various services and resources to improve their quality of life. This role requires strong interpersonal skills, a deep understanding of veterans’ issues, and the ability to coordinate with multiple agencies and service providers. Case manager positions that require travel within Escambia, Walton, Okaloosa, and Santa Rosa counties.

Responsibilities:

• Assess clients’ needs, strengths and barriers and work with clients to develop effective housing stability plans.

• Provide effective case management including regular home visits, housing counseling, client-centered advocacy, referrals, crisis intervention etc. to create housing stability.

• Actively manage assigned caseload of veterans and/or veteran families, including traditional, Shallow Subsidy and HUD-VASH cases

• Effectively communicate the Priority Veteran program to prospective clients, landlords and community, and conduct outreach to assigned counties

• Maintain consistent contact with local homeless coalitions (COC’s), VA Hospital, shelters, etc.

• Follow up on all inquiries in a timely manner, and accurately conduct eligibility determinations following grant guidelines.

• Maintain accurate and timely case documents

• Accurately follow the grant regulations and program policies.

• Actively participate in professional development training.

• Actively participate in weekly staff meetings and individual supervision

Requirements:

• Case management experience, including knowledge of trauma-informed care and person-first practices

• Excellent interpersonal and communication skills

• Excellent computer skills to include Microsoft Office suite and Client Management System

• Valid Driver's License & Reliable Transportation (Willing to travel locally 70% of the time)

• Able to establish rapport with clients who are homeless or facing homelessness

• Background checks to include criminal record, drug screen, and TB test

Plusses:

• Bachelor’s Degree in Social Work or comparable area preferred

• Familiarity with Housing First, Veteran’s Administration (VA) and/or US Military a plus

Compensation: $19.25/hr to $21.15/hr.

Exact compensation may vary based on several factors, including skills, experience, and education. Benefit packages for this role will start on the 31st day of employment and include medical, dental, and vision insurance, as well as HSA, FSA, and DCFSA account options, and 401k retirement account access with employer matching. Employees in this role are also entitled to paid sick leave and/or other paid time off as provided by applicable law.
